Sports Physicals

Sports Physicals are available at Summit Medical with Dr. Jason Carlson

Summit Medical is located at 150 North Washington Blvd  (Directly East of Rite Aid) 801-782-0123.

Their office hours are M-F 9:00 a.m.-8:00 p.m. Sat & Sun 12-5.

Dr.  Carlson is a friend of Michelle Christensen a fremont lacrosse mom.  He is offering a Sports Physical to our players.

Physical Cost is only $20.  If you tell them you are with Fremont Lacrosse, he will donate $10 from the $20, back to the team.
