REGISTRATION

Registration information for the High School Regular season: 

All of the following are required for participation in the Fremont Lacrosse Club:

1.  Register with the Fremont Lacrosse Club.  Registration packets will be passed out at practice in Jan. Players must turn in all forms to participate in practice.  These forms are required to be eligible to play lacrosse in this league.  Fremont Lacrosse team fee is $80.  New players must also purchase 2 pair of shorts for $50.

2.  Register with U.S. Lacrosse at www.uslacrosse.org.  Click on "Membership Info", Then "Click Here to Apply or Renew Online".  register as a high school player. You do not have to pick a team to be associated with.  The membership fee is $35.00 payable with a credit card and is valid for 1 year.  Write down your number, you will need it for the next step.  You may also look up your current membership number and expiration date at this site.

3.  Register with Utah Lacrosse at www.utahlax.org.  You will need your U.S.Lacrosse membership number, expiration date and Health Insurance Policy number. Click on "High School Registration", then "Boys High School ALL PLAYER and Program (Team) Registration".  Sign and agree to waivers. On the next page select Fremont as the team in the drop-down menu.  You only need to fill in the sections with a red *.  Enter, this will take you to the next page to enter your credit card billing information and pay.  Save your receipt as confirmation.  Player Registraion is $112.00. 

  Scholarships are available for the $112 Utah Lacrosse fee.  Scholarships are fee waivers for players that would be unable to play without financial assistance.  Requests must be submitted in writing to the Fremont Lacrosse Board. The deadline to request the scholarship is Jan 30.

5.  It is now required by the Utah Lacrosse Association that all players obtain a physician’s certificate stating that they are physically able to compete in inter-school athletic contests. Players must turn in written proof of their physical to Board Members or Sheridan Moore, team manager, to participate in games.
